Client Overview
Multi-Location Clinic Network
The Challenge
The clinic was using three different disjointed systems for booking, patient records, and payments, leading to administrative chaos and a poor patient experience.
Our Solution
We developed a unified cross-platform mobile application utilizing Flutter and Node.js. It features a secure patient portal, real-time doctor availability calendars, automated SMS/WhatsApp reminders, and a frictionless payment gateway.
The Results
- No-show rate reduced by 48%
- Administrative workload reduced by 30%
- Processed ₹1.2Cr+ in secure online payments
- 98% patient satisfaction rating on the new app
Three Systems, Zero Communication
This multi-location clinic network (5 locations across two cities) had a classic integration nightmare:
- Booking: A shared Google Calendar (per doctor) managed by reception staff
- Patient records: Paper files in physical cabinets + a legacy desktop app from 2015
- Payments: Cash-only with manual receipt books
The result? Double bookings happened weekly. Patient files got lost between locations. No-shows ran at 22% because reminder calls were inconsistent. And the admin team spent more time managing systems than managing patients.
Discovery & Architecture (Week 1-2)
We spent the first week shadowing the clinic operations — watching receptionists juggle phone calls, WhatsApp messages, and walk-ins simultaneously. This revealed requirements that a standard SaaS booking tool couldn't solve:
- Doctors needed different slot durations per appointment type (15 min for follow-ups, 30 min for new patients, 45 min for procedures)
- Reception needed to manage walk-ins alongside pre-booked appointments
- Patients needed to book across any location for any available doctor in their specialty
- The billing system needed to handle partial payments, insurance, and family accounts
The Build
Patient Mobile App (Flutter)
A cross-platform app (iOS + Android from a single codebase) that lets patients:
- Browse doctors by specialty, location, and availability
- Book appointments with real-time slot availability (no more "call us to confirm")
- Receive smart reminders via WhatsApp/SMS at 24 hours, 2 hours, and 30 minutes before
- Pay online via Razorpay (UPI, cards, net banking) or mark as "pay at clinic"
- View medical history including past visits, prescriptions, and test results
- Family accounts — one login managing appointments for spouse, children, and parents
Clinic Admin Dashboard (Web)
A responsive web dashboard for reception and operations staff:
- Daily schedule view with drag-and-drop rescheduling
- Walk-in queue management with estimated wait time display
- Patient search across all locations with instant medical history access
- Automated no-show handling — if a patient marks "running late," the system adjusts the queue. If they don't show, it auto-triggers a rebooking WhatsApp message
- Revenue dashboard with daily/weekly/monthly breakdowns by doctor, location, and service type
Reminder Pipeline
The automated reminder system was the single biggest driver of no-show reduction:
- 24-hour reminder (WhatsApp) with one-tap buttons: "Confirm" / "Reschedule" / "Cancel"
- 2-hour reminder (SMS) with Google Maps link to the clinic
- 30-minute reminder (push notification) with parking information
- No-response escalation — if a patient doesn't confirm 4 hours before, the slot opens to a waitlist
This pipeline reduced no-shows from 22% to 11.5% — a 48% improvement that directly translated to recovered revenue.
Infrastructure
- Firebase for real-time database sync across locations
- Node.js backend on Google Cloud Run for business logic and payment processing
- Razorpay payment gateway with auto-reconciliation
- Health data encryption (AES-256) for all patient records
Post-Launch Impact
Within 90 days, the platform processed over ₹1.2 Crore in online payments — revenue that previously required patients to carry cash. The 98% patient satisfaction rating (surveyed via in-app feedback) validated the UX investment.
But the most meaningful metric was staff relief. Receptionists reported that their administrative workload dropped by roughly 30%, freeing them to focus on patient experience instead of system management. One receptionist told us: "I used to spend my entire morning on the phone confirming appointments. Now I actually get to greet patients when they walk in."
"This platform transformed our operations. Patients love the convenience, and our staff finally has a system that actually works."
